Regarding the confirmation of identity as the person in question or as a representative

For requests regarding the notification, disclosure, correction, addition, or deletion, suspension of use, or erasure (hereinafter collectively referred to as "disclosure, etc.") of personal data held by our group, please fill in the required information on the designated personal data disclosure request form of our group and mail it to us. Please note that we cannot accept requests made by visiting our office, phone, fax, email, or other methods, so we kindly ask for your understanding.

How to claim

Please attach the documents described in 1. or 2. below to the request form for disclosure of retained personal data.
The personal identification documents and agent identification documents you have sent will be destroyed within six months.

  • In the case of a request by the person themselves (identity verification documents): One of the following documents.
    ① Copy of driver's license
    ② Copy of passport
    ③ Copy of health insurance card
    ④Original copy of resident's card
  • In the case of a claim by a representative (representative verification documents): All of the following ① to ③.

    ② A document confirming that the person is the agent themselves (one of the documents ① to ④ mentioned above regarding the agent)
    ③Documents confirming the authority of representation
    In the case of a legal representative: Document to confirm the existence of legal representation authority (one copy of the family register extract)
    In the case of a voluntary agent: A power of attorney with the registered seal and the certificate of seal registration of the principal

Fees and Collection Methods

For requests for disclosure of retained personal data, we will respond in writing, so please enclose 940 yen worth of postage stamps (110 yen for standard mail, 480 yen for registered mail, and 350 yen for delivery certification) with the request documents.
If the postage is insufficient or not enclosed, we will inform you of this. However, if payment is not made within the specified period, it will be considered that no request for disclosure, etc., was made.
